To facilitate your check in process, please take time to fill out the developmental screening questionnaire on GATEWAY prior to your visit today.  if you do not yet have a GATEWAY account, please arrive a few minutes early so that you can fill out the questionnaires in the office.

What to expect during this visit:

During this visit we will weigh and measure your baby,  do a full examination, discuss feeding, development and sleep and answer any questions that you may have.

The routine vaccinations for this visit are:

• Vaxelis - Combination of DTaP, IPV, Hib, and HBV (Hepatitis B virus) vaccine

• PCV-20 - Pneumococcus 20-valent vaccine
• Rota Teq - Oral rotavirus vaccine
